Error message on image upload

Please describe in detail the issue you are having:
On testing placing an ad, at step 4 the process goes through, but I get this error message:
Notice: Undefined index: category in /home/jessiel1/public_html/'domain'/includes/core/checkout/step5.php on line 25
Warning: Cannot modify header information - headers already sent by (output started at /home/jessiel1/public_html/'domain'/includes/core/checkout/step5.php:25) in /home/jessiel1/public_html/'domain'/includes/core/checkout/step5.php on line 144

Also all the content text and navigation boxes text has changed..the sizes are very large. Everything else on the page seems ok.

Any suggestions as to what the problem might be?
