Created extra fields and designated then as searchable.
Are these fields suppose to show up on the advanced search page?
If so, they are not doing so and what can I do to make them appear in search form?

Yes they should show on the advanced search page. However since extra fields are associated with categories they will not display until a category is chosen first.
