What version of 68 Classifieds are you running?
v4.2.0 Developer

What template are you using?
purple(modified)

Please describe in detail the issue you are having:

I noticed that when a user goes to search for an item, the location being searched (city and state and country) are restricted by the locations of registered users that posted an ad. How can I modify the search settings to where all city, states, and countries will show in the search locations even if there is no ad posted from their region.
I think this confuses users when they search because they think that the the city,state, and country fields define where they can only post ads.
My site www.flemzy.com is geared toward the whole world, that is why I would like this change somehow.

Peronally id get miffed and walk away from your site if I did a bunch of searches on areas where no ads existed. Its oinly confusing if you dont explaine it to your users. Add some text to the search and explain the dropdowns. Otherwise you can modify the template and just put in text fields instead of dropdowns. That in and of itself removes any referential integrity however even if an ad exists in a given place.

What file do I modify in order to put text on the search.php page. If it is the search.php page, then how do I do the coding without a syntax error.

Seems like you need to read the 68C documentation and other info on the forum about how to modify your templates. Seriously, invest an hour in doing this. It will pay dividends.
