The SEO module will work with either version but it does require your hosted on a system running apache with mod_rewrite enabled.

Is the SEO mod included with the hosted version? What version is the hosted package anyway? Is it already v4?

The SEO mod is a seperate purchaseable module.

Since V4 is in Beta it doesnt make sense that itwould be installed as your live site. However as part of the hosted version Im sure you have access to the V4 code via your customer area on the 68C site.
